Cabinet asked to back transformative social care projects

Flintshire County Council is taking forward three new social care initiatives aimed at transforming the way children, young people and adults are supported.

The council’s Cabinet will be asked to endorse each of the projects next week after they were supported by Social and Health Care Overview and Scrutiny Committee.

The first report, ‘Right Care, Right Place’, sets out plans to roll out a new ‘Progression Model’ for adults with learning disabilities living in supported housing.

This innovative approach builds on individuals’ strengths and aspirations, empowering them live as independently as possible. It ensures care provided is proportionate and personalised, taking into account the strengths and support needs of the household as a whole.

The council’s early work on the Progression Model was recognised with a Social Care Wales Accolade this year.

Meanwhile the council is also progressing a new approach to help reduce the number of looked after children in costly out-of-county residential homes.

The project, in partnership with the Reconnect Programme, supports children and young people to transition from residential placements to family-based or semi-independent settings, where it is in their best interests to do so.

A separate report on expanding Flintshire’s in-house fostering service will also be considered. It sets out three areas of focus, including the targeted recruitment of specialist foster carers to support the Reconnect Programme.

It also sets out plans to expand both the Special Guardianship Order service and the Mockingbird service – the first of its kind in Wales – which sees dedicated fostering households act as ‘hubs’ to support other foster carers.

Councillor Christine Jones, Cabinet Member for Social Services and Wellbeing, said: “These initiatives reflect our absolute commitment to supporting people in ways that are compassionate, empowering, and ultimately help them achieve better outcomes.

“While they will help us manage increasing budget pressures, the most important impact is the difference these programmes will make to people’s lives.”

Cabinet will meet next Tuesday, December 16, to approve the plans.
