Blue Badge - Eligibility Checker Discretionary Criteria
You can apply by contacting Customer Services on 01352 701304 or by visiting your nearest Flintshire Connects office. You should bring the following documents with you:
Proof of Identity
You will need to provide one of the following:
- Birth/Adoption certificate
- Marriage/Divorce certificate
- Civil partnership/Dissolution certificate
- Valid driving license
- Valid passport
Proof of Residency
There are a few ways to prove your residency, these are:
- Council tax bill, or
- Consent for us to check our council tax records/electoral register/school records on your behalf
Photograph for the Blue Badge
This should be a recent passport-style photograph of the applicant. The photograph should show the applicants full face so that the holder can be easily identified and no one else should be in the photograph. The photograph will be placed on the back of the badge and will not be visible when the badge is being displayed in the vehicle. A photograph can be taken by one of our customer advisors at our Flintshire Connects centres.
Proof of Impairment
The evidence you need to provide varies between different impairments. Below are a list of impairments with the related evidence explained:
Eligibility Criteria
Permanent and substantial disability which causes inability to walk of very considerable difficulty in walking
Evidence
Please provide evidence in the form of medical reports, supporting consultant letters, consultant hospital appointments, prescriptions, adaptations and walking aid receipts as proof of:
- Excessive pain during and after walking
- Breathlessness
- Distance you can walk
- Length of time you can walk
- Posture, rhythm, co-ordination, balance and stride
- Use of walking aids and oxygen
- Outdoor walking, if different
Other Evidence
- Receipts for adaptations (stair lifts, hand / grab rails, walk in showers, bath lifts and ramps)
- Vehicle adaptations (please provide insurance policy as confirmation)
Eligibility Criteria
You must drive a vehicle regularly, have a disability in both arms and is unable to operate, or has considerable difficulty in operating, all or some types of parking equipment.
Evidence
Consultant reports / letter and driving licence - must be coded as either of the follow:
- 40 - Adapted steering
- 79 - Restricted to certain vehicles

You have a special case terminal illness and a mobility impairment
Evidence
You will need to produce an original DS1500 form and a supporting letter from McMillan nurse or relevant health specialist.
Eligibility Criteria
You must have or care for a child who, on account of a condition, must always be accompanied by bulky medical equipment which cannot be carried around with the child without great difficulty
Evidence
A letter from a paediatrician outlining the medical condition. Also evidence to confirm use of any of the following:
- Ventilators
- Suction machines
- Feed pumps
- Parenteral equipment
- Syringe drivers
- Oxygen administration equipment
- Continuous oxygen saturation monitoring equipment
- Casts and associated medical equipment for correction of hip dysplasia
Eligibility Criteria
You must have or care for a child who, on account of a condition, must always be kept near a motor vehicle so that, if necessary, treatment for that condition can be given in the vehicle or the child can be taken quickly in the vehicle to a place where such treatment can be given
Evidence
Medical records in relation to the condition. Examples of these conditions would be:
- Tracheostomies
- Severe epilepsy/fitting
- Highly unstable diabetes
- Terminally ill children who can only access brief moments of outside life and need a quick route home
